REQUEST FOR CLEARANCE TO BOX OUTSIDE OF QUEENSLAND

All Boxing Qld members that wish to compete outside of Qld must apply for a clearance to do so.

All information needs to be completed in full.

Please note,

if you want to compete in NSW, boxers serology's must be current within 6 months, all other states are 12 months.

Coaches must have a min Bronze level accreditation and be registered as a coach with Boxing Qld.

Please list all Boxers full names with their BQ member number, Date of Birth, Gender & current Age

MEDICAL*

List all Boxers names and the date they completed their current medical

SEROLOGY*

List all boxers' names and the date they completed their current serology for the boxers that require a serology (all boxers U17 divison and over), if a serology is not required please list N/A

ATTENDING TOURNAMENT INFORMATION*

Provide the date, Location (address) & state of the competition

COACH*

Add all travelling coaches' names, BQ member number, coach accreditation level (bronze/Silver) & accreditation number
